Consent to Treat
This page describes the general informed-consent framework Melbi will use when clinical services become available. A treatment-specific informed consent will be presented to you at the time of any individual prescription.

Informed consent generally
Informed consent means that, before treatment is provided, you have the opportunity to understand the nature of the proposed treatment, its risks and benefits, reasonable alternatives, and the risks of declining treatment. You will be asked to acknowledge this understanding electronically before any prescription is issued.
Eligibility review
Eligibility is determined by a licensed clinician on a case-by-case basis. The clinician may approve, deny, request more information, or require a brief live phone or video visit. Approval is never guaranteed, and not all patients qualify.
Compounded medications
If a compounded medication is recommended, you will be informed at the time of prescription that compounded medications are not FDA-approved and that the FDA does not review compounded medications for safety, effectiveness, or quality before they are marketed. You will be asked to acknowledge this disclosure before treatment proceeds.
Right to refuse
You may decline any treatment at any time. Your clinician may also decline to prescribe at any time if treatment is not clinically appropriate or if you do not provide accurate information.
Risks of declining
If you decline a recommended treatment, your clinician will discuss the potential risks of declining and any reasonable alternatives.
Refills and ongoing follow-up
Refills require continued clinical appropriateness review. Ongoing follow-up, including lab work or check-ins, may be required for certain wellness categories. Failure to complete required follow-up may result in discontinuation of treatment.
Adverse events
If you experience an unexpected reaction or adverse event, contact your clinician through the Melbi messaging system. For emergencies, call 911 or go to your nearest emergency department immediately.
Honest disclosure
You are responsible for providing complete and accurate information about your medical history, current medications, allergies, and lifestyle. Providing inaccurate information can put your safety at risk and may result in denial or discontinuation of care.
Subscription and cancellation
When services launch, treatment programs may be offered as auto-renewing subscriptions. Subscription terms, billing cycles, refund policies, and cancellation procedures will be presented at checkout. You may cancel any subscription at any time, subject to the terms disclosed at that time.
